If something about your care did not go the way it should have, we want to hear about it. Feedback is how we find the gaps in what we do.
How to raise it with us

It helps if you include the date of your consultation, what happened, and what you would like us to do about it. You can ask a family member, carer or advocate to raise it for you.
What happens next
- We acknowledge your complaint within 2 business days of receiving it.
- We look into what happened. That may mean reviewing your file, and speaking with the practitioner involved.
- We aim to give you a written response within 30 days. If it will take longer, we tell you why, and how long we expect it to take.
- We tell you what we found, and what we are changing as a result.
Making a complaint does not affect your access to care with us. Your complaint is recorded separately from your clinical record.
If you are not satisfied with our response
You can go to an external body at any time. You do not have to come to us first.
